Common Employer Questions & Answers

Q. How much does the Capstone Health Network program cost?
A. The cost for businesses and employers is just $12.95 per employee per month for our Individual Plan and $18.95 per employee per month for our Family Plan.  Employers are free to mix and match these programs.  For example, you could choose to provide your managers with a Family Plan but only make the Individual Plan available to your non-management employees.  Or, you could elect to provide your employees with our Individual Plan but inform them they can choose to have you deduct $6 from their pay every month to make up the difference between an Individual and Family Plan.

Q. Do I need to sign a long term contract?
A. No.  You are free to continue paying for the Capstone Health Network program for as long or as short as you would like on a month to month basis.

Q. When will my credit card be charged?
A.  Your credit card will be charged on the 1st of every month until you inform us you wish to stop the Capstone Health Network program.

Q. How can I can cancel the Capstone Health Network program?
A. Simply inform us by not later than the 15th of a month and we will stop billing you for our program as of the 1st of the next month.  For example: If you inform us on or before October 15th that you want to stop the Capstone Health Network program we will cancel your program and not bill you starting on November 1st.  If, for example, you notify us on October 25th, you will be billed one final time for November and your cancellation will go into effect on December 1st.

Q. Can I change which employees I want to cover?
A.  Yes.  You can add or remove any employee at any time.  Simply inform us by the 15th of a month and we will only bill you for the employees you want covered starting on the 1st of the following month.  For example: suppose you initially sign up to have Capstone cover 10 employees but 2 of the covered employees leave your company.  If you inform us which employees you no longer want Capstone to cover by the 15th of October, we will only bill you for the remaining 8 employees on November 1st.  If, for example, you notify us you want to reduce the number of your covered employees on October 25th, you will be billed for 10 employees for November and the reduction to 8 employees will go into effect on December 1st.
