12 Month Plan:	One payment of $142.50 is due when you sign up. Pay for 11 months and get month FREE!	One payment of $208.50 is due at the time you sign up. Pay for 11 months and get month FREE!

To become a Member of the Capstone Healthcare Services program, please provide your name, email, and Order Code (if known) and then click the “Sign Up Now” button. Once you have selected the type of membership that best meets your needs and provided us with standard credit card information, you will have successfully joined the Capstone Healthcare Services family!

Signing up to become a Member is quick and easy and there are no health questions or exclusions due to pre-existing conditions. Becoming a Member will take only a few minutes of your time.  Once you become a Member we will send you your New Member package which includes your Capstone Health Network Membership Card and information about how you and/or your family can use and benefit from our services.
