Important CapstoneCare Sign-Up Instructions

Thank you for your interest in being a member of the Capstone Health Network family! Signing up for our CapstoneCare™ healthcare from home program is fast, easy, and takes just minutes. There are no questions about your health, citizenship, or personal information, and we don’t require your social security number. To sign up, choose the Plan below that best fits the needs of you and your family. Then, click on the appropriate color sign-up button and follow the simple sign-up instructions.

After you complete the sign-up process and provide your credit card, we will email you a Welcome Letter with information about your new healthcare program and a CapstoneCare Membership Card which you can keep in your wallet or purse. In most cases, your membership will be active and ready to use in 3 or 4 business days.

Minimum Membership Period. The Minimum Membership Period is 2 months after which you can cancel your membership at any time by giving us a minimum of 10 days written notice via an email sent to [email protected].
