Sign Up Process and Answers to
Common Employer Questions

Employer Sign Up Process

The sign-up process to provide the Capstone Health Network program to your employees takes just minutes.  Simply go to the Order Now section and select Businesses/Employers.  You will be prompted to provide the following:

Step 1.  Employee Information.  Provide basic information about each employee you want Capstone Health Network to cover: first name, last name, email address, and the type of plan you want to provide.  You will begin by listing information for all employees you want to cover with the Capstone Individual Plan ($12.95 per employee per month).  Then you will provide the names and basic information for any employees you want to cover with the Family Plan ($18.95 per month).  Note: You are free to purchase a mix of Individual and Family Plans and you can choose to cover some or all of your full time, part time, or 1099 employees.  Once you provide this information the system will calculate and display the amount of your monthly payment.

Step 2.  Employer Information.  Next you will be asked to provide basic information such as your company or employer name, address, contact person, and their phone and email.  You will also be asked to enter the “Order Code” associated with the person or group who introduced you to the Capstone Health Network program.  This is very important to enter if you know it.

Step 3.  Credit Card Information.   To protect your personal and credit card information you will be transferred to a secure payment portal.  Capstone accepts Mastercard, VISA, PayPal, and American Express.

Step 4.  Once you complete the sign-up process, your credit card will be charged on the 1st of the next upcoming month which will be the date on which your program will be activated.  For example, if you sign up for the Capstone Health Network program in September your card will be charged on October 1st and your program will be active as of that date.

Common Employer Questions and Answers

Q. If we decide to sign up for the Capstone Health Network program when will our program begin?

A. Once you sign up for the Capstone Health Network program your credit card will be charged on the 1st of the upcoming month.  For example, if you sign up for the Capstone Health Network program in September your card will be charged on October 1st and your program will be active as of that date.  Capstone will email each covered employee a Welcome Letter, a description of their new health benefits, and a Membership Card which they can keep in their wallet or purse.

Q. How much does the Capstone Health Network program cost?

A. The special price for businesses and employers is just $12.95 per employee per month for the Capstone Individual Plan and $18.95 per employee per month for our Family Plan.  Employers are free to mix and match these programs.  For example, you could choose to provide your managers with the Capstone Family Plan but only provide the Individual Plan to your non-management employees.  Or, you can provide your employees with our Individual Plan but inform them that at their option they can choose to have you deduct $6 from their pay every month to make up the difference between an Individual and Family Plan.

Q. Do we need to sign a long term contract?

A. No.  You are free to continue paying for the Capstone Health Network program on a month to month basis for as long as you want.

Q. When will my credit card be charged?

A. Your credit card will be charged on the 1st of every month until you inform us you wish to stop the Capstone Health Network program.

Q. If we don’t decide to provide the Capstone Health Network program can our employees sign up and pay for the program on their own?

A. Yes. We can provide you with promotional literature about the Capstone Health Network program which you can email or share with your employees. Employees who wish to sign up on their own can go to our website (www.CapstoneHealthNetwork.com) and visit the Order Now section.  They will select the Association/Affiliate option which will allow them to sign up and pay for the program themselves.

Q. How can we cancel the Capstone Health Network program?

A. Simply inform us by not later than the 20th of a month by sending an email to cancel@capstonehealthnetwork.com.We will stop billing you for our program as of the 1stof the next month.  For example: If you inform us on or before October 20ththat you want to stop the Capstone Health Network program, we will cancel your program and stop billing you starting on November 1st.  If you notify us between October 21st and October 31st, you will be billed one final time for November and your cancellation will go into effect on December 1st.

Q. Can we reduce the number of employees we want to cover?

A. Yes.  You can stop paying for any employee you no longer want to cover.  Simply inform us by the 20thof a month by sending us an email to changemyplan@capstonehealthnetwork.com.  We will not bill you for that employee starting on the 1stof the following month.  For example: suppose you initially sign up to have Capstone cover 10 employees but 2 of the covered employees leave your company.  If you inform us which employees you no longer want Capstone to cover by the 20th of October, we will only bill you for the remaining 8 employees on November 1st.  If you notify us that you want to reduce the number of your covered employees between October 21st to October 31st, you will be billed for 10 employees for November and the reduction to 8 employees will go into effect on December 1st.

Q. How can we add a new employee or employees we want to cover?

A. Simply inform us on or by the 27thof a month and we will add the new employee(s) to your list of covered employees as of the 1stof the upcoming month.  For example: suppose you initially sign up to have Capstone cover 10 employees but you wish to add 3 more employees.  If you inform us which new employees you want to add on or by the 27th of October, you will be billed for these new covered employees starting on November 1st and their coverage will begin on that day.  If, for example, you notify us that you want to add an employee or employees between October 28th and October 31st, you will be billed for these new employees on December 1st and their coverage will begin on or near that day.

Q. How will our employees know what their coverage is and when it begins?

A, After you sign up and pay for the Capstone Health Network program, Capstone will send each covered employee an email explaining the benefits of the program.  The email will also include their Capstone Membership Card which your employees can print out and keep in their wallet or purse.  For employees for whom you did not provide an email, within a few days of their program activation date we will email your company a generic Welcome Letter and their Capstone Membership Card which you can print out and give to each such employee.
